New patients welcome

Register as a patient

We are currently accepting new patients from Kilmessan and the surrounding area. Download the practice registration form below, fill it in, and bring it to the surgery — or phone us and we will take your details over the phone.

Step one

Get the registration form

The form is one page. It asks for your personal details, an emergency contact, your previous GP, your medical card details if you have one, and a short summary of your medical history.

Print it and fill it in at home, or collect a copy at reception and complete it while you are with us — whichever suits you better.

The Kilmessan General Practice patient registration form, showing sections for personal details, emergency contact, previous GP, medical cover, medical information and consent
One form per patient. A parent or guardian completes it for anyone under 16.

Step two

Three ways to register

Whichever you choose, we will ring you to confirm your place and arrange your first appointment.

Bring it in

Drop the completed form in to the surgery during opening hours, or bring it with you to your first appointment. This is the simplest option, and it means we can check everything with you there and then.

Register by phone

No printer, or would rather just talk to someone? Ring the surgery and we will take your details over the phone and have the form ready for you to sign when you come in.

Call 089 483 7929

Email it to us

Scan or photograph the completed form and email it to the practice. Please put your name and date of birth in the subject line so we can match it to your record.

Email the practice

A note on sending your details

Ordinary email is not encrypted. If you would rather not send your PPS number, medical card number or medical history that way, leave those parts of the form blank and fill them in with us at reception — or simply hand the form in. Your information is stored and used in line with GDPR and our privacy notice. Please never use email to report an urgent medical problem: phone 089 483 7929 instead.

What to bring to your first appointment

Please bring the following with you:

  • Your completed registration form
  • Photo ID (passport or driving licence)
  • Proof of address, if your ID is out of date
  • Your Medical Card or GP Visit Card, if you have one
  • Your PPS number
  • A list of any medication you take
  • For children: birth certificate and vaccination record

Step three

What happens next

  1. We review your form

Usually within one or two working days of receiving it.

  2. We ring you

To confirm your registration and book your first appointment with Dr Mirza.

  3. We request your records

With your written consent on the form, we write to your previous practice for a copy of your notes. This can take several weeks, so please bring a list of your current medication to your first visit.

  4. You are on our list

If you hold a Medical Card or GP Visit Card, we will also give you a change of GP form to sign so the HSE can transfer it to this practice.

Common questions

Registering with us

Do you have space for new patients?

Yes. We are accepting new patients living in Kilmessan and the surrounding area. If you live further afield, get in touch and we will let you know whether we can take you on.

Can I register my whole family at once?

Please complete a separate form for each family member, including children. A parent or guardian should complete and sign the form for anyone under 16.

I do not have a printer — what should I do?

Either call in to the surgery and we will give you a blank form at reception, or ring us on 089 483 7929 and we will take your details over the phone and have the form ready for you to sign.

How do I transfer my Medical Card or GP Visit Card to this practice?

Tell us you hold a card when you register and we will give you a change-of-doctor form to sign. We send it to the HSE on your behalf; the change usually takes a few weeks to come through. Until then you may be asked to pay privately, and we will refund you once the transfer is confirmed. The form is also on our forms page.

What happens to my records from my old GP?

With your written consent we write to your previous practice and ask them to forward a copy of your notes. This can take several weeks, so please bring a list of your current medication to your first appointment.

I am only visiting the area — can I be seen?

Yes. We see temporary and visiting patients on a private basis without full registration. Phone the surgery on 089 483 7929 to arrange an appointment, and see our fees.

Any other questions about joining the practice? Ring us on 089 483 7929 or email kilmessangp@gmail.com.
