Forms
Our own practice forms are here to download. Below them are the official HSE, social welfare, driving licence and Revenue forms our patients most often need — each one links straight to the source, so you always get the current version.

About the links below
Everything from here on is hosted by the HSE, the Department of Social Protection, the NDLS or Revenue, and opens in a new tab. We link to the official pages rather than storing our own copies, so you always get the current version of a form. If a link has moved, please tell us and we will fix it.

Transferring to us
Changing your doctor
If you hold a Medical Card or GP Visit Card and want to move it to this practice, you need to complete a change of GP form. We will sign it for you at reception — just ask.
- Change of GP Form (Medical Card / GP Visit Card) Complete it, we sign it, then it goes to the HSE Eligibility Unit. The transfer usually takes a few weeks.
- Change your medical card details Update your address, add a family member, or report a lost card.
New to the practice?
Complete our patient registration form first — the change of GP form transfers your card, but the registration form is what puts you on our list and lets us request your notes from your previous doctor.
Department of Social Protection

- Maternity Benefit Apply at least 6 weeks before your leave starts (12 weeks if self-employed). Form MB3 is the part your doctor completes.
- Maternity Benefit paper form (MB1) If you would rather not apply online.
- Disability Allowance Weekly payment for people aged 16 to 66 with a disability. Includes a medical report your GP completes.
- Carer's Benefit For people who leave work to care for someone full time. PRSI-based.

Forms we complete for you
How do I get a form signed or completed?
Book an appointment and bring the blank form with you. Please do not leave forms at reception expecting them the same day — most need the doctor to examine you or to review your record properly.
How long will it take?
Forms completed during a consultation are done there and then. Reports that have to be written up afterwards — insurance and solicitors' reports in particular — usually take up to two weeks, and longer if we have to request hospital records.
Is there a charge?
Yes for most forms. Medicals, certificates and third-party reports sit outside the GMS scheme, so they are charged privately to everyone including Medical Card holders. Illness Benefit certificates and Maternity & Infant Care Scheme visits are free. See our fees, and ask us for a quote before you book if you are unsure.
Can you complete a form for someone else in my family?
Only if they are a registered patient here and, where they are an adult, only with their consent. A parent or guardian can complete forms on behalf of a child under 16.
